How do I get started/schedule an appointment?

You can schedule an appointment online, by email, or by calling 317-455-LMHC. All new clients will be asked to complete two forms: the informed consent contract and a brief intake form which can be completed online or printed. Copies of all forms/documents can be found here. These can also be completed at your first session. 

Can I schedule a teleconference? Do you do online therapy?

A free 15 minute teleconference can be scheduled to determine if I can help you and we would be a good fit to work together. Established clients can schedule a teleconference by phone or videoconference via Doxy.Me. It is important to recognize that there may be a limit to confidentiality when using such technology, depending on your location. Clients who live outside of the Indianapolis Metro area find this to be a helpful option for scheduling appointments. Potential clients outside of the state of Indiana may contact me to determine if teleconferencing would be appropriate and if I can be licensed in your state. Click here to schedule a teleconference or videoconference. A credit card authorization form will need to be on file before teleconference sessions can be scheduled.  

Is my insurance accepted?

You are welcome to use your Health Savings Account (HSA) to pay for appointments; however, I do not bill insurance companies directly. Fees for the majority of services are determined by a sliding-scale based on income. This allows you to direct your own treatment and allows more time to be spent in direct interaction with clients, rather than billing and answering to insurance companies. I have been approved by multiple insurance companies in the past, and you are welcome to request reimbursement from your insurance carrier; information to assist in your appeal will be provided at your request. What is the cost of counseling?

The sliding-fee scale used to determine rates is based on annual household income. You will need to complete a Fee Reduction Contract and supply financial documentation. Payment is expected at the time of service. Cash, check, charge, and HSA are all acceptable forms of payment. 

May I communicate with you via email?

Email should primarily be used for appointment re-scheduling and is not the media for sensitive, therapeutic material. If you choose to communicate by email at some point in our work together, please be aware that email is not completely confidential. All emails are retained in the logs of the internet service providers from which it is sent and received. While under normal circumstances these logs are not viewed, they are, in theory, available to be read by the system administrator(s) of the internet service provider. If more than ten minutes per week is spent reading and responding to your emails, I reserve the right to bill this time on a prorated basis.

What is the cancelation policy?

Appointments must be canceled 24 hours in advance; if you fail to cancel any appointment with at least 24 hours notice, you will be responsible for half of the meeting fee, expected at the next scheduled meeting. Emergency situations do occasionally arise, and this failed appointment fee may be waived at my discretion. Every effort will be made to respect your time to the same degree that has been requested of you.

What is the inclemet weather policy?

If you are unable to make it into my office due to snow or ice, we can simply switch your appointment to a videoconferencevor teleconference. I will plan to be in my office every day that it is safe to be there.  

What if I have an emergency or feel suicidal?

If you are feeling suicidal, support is available 24 hours/7 days per week at 1-800-273-TALK (8255). Please contact 911 or your nearest emergency room for immediate assistance. For crisis situations that do not include an immediate medical or safety concern, please call 317-455-LMHC. If I am not available, please leave a message, and I will follow up as soon as I am able. 

What are the limits of confidentiality? Will my information ever be shared with anyone? 

Your confidentiality is always a priority in this process. Your identity, personal information, and the topics you discuss will be protected with the highest standards available. Without your written permission, information about you shall not be shared. Even after written permission is granted, it may be revoked at any time. If there is concern of danger or harm to yourself and/or others, I am legally required to report this information to the authorities to ensure the safety of all. In the case that you are threatening harm to yourself, I may be obliged to contact loved ones to provide protection and/or seek more intensive treatment. I may also be required to take protective actions if it is believed that you are threatening harm to another. These actions may include notifying the potential victim, contacting police, or more intensive treatment. I am obliged by law to report any suspicion or allegation of abuse or neglect of a child, elderly or disabled person.
