FAQIs my insurance accepted?

Fees for the majority of services are determined by a sliding-scale based on income. This allows you to direct your own treatment and allows more time to be spent in direct interaction with clients, rather than billing and answering to insurance companies. Brooke Randolph has been approved by multiple insurance companies in the past, and you are welcome to request reimbursement from your insurance carrier; information to assist in your appeal will be provided at your request.

What is the cost of therapeutic services?

The sliding-fee scale used to determine rates is based on annual household income. You may be asked to provide copies of income tax returns or check stubs to secure the reduced rate. May I communicate with you via email?

Email should primarily be used for appointment scheduling and is not the media for sensitive, therapeutic material. If you choose to communicate by email at some point in our work together, please be aware that email is not completely confidential. All emails are retained in the logs of the internet service providers from which it is sent and received. While under normal circumstances these logs are not viewed, they are, in theory, available to be read by the system administrator(s) of the internet service provider. If more than ten minutes per week is spent reading and responding to your emails, Brooke Randolph reserves the right to bill this time on a prorated basis.

What is the cancellation policy?

Appointments must be cancelled 24 hours in advance; if you fail to cancel any appointment with at least 24 hours notice, you will be responsible for half of the meeting fee, expected at the next scheduled meeting. Emergency situations do occasionally arise, and this failed appointment fee may be waived at the discretion of Brooke Randolph. Every effort will be made to respect your time to the same degree that has been requested of you.

What if I have an emergency or you are not available?

In the case of an emergency, please call 765-623-5656. If not available, please leave a message for Brooke Randolph to follow up with you regarding your emergency. Contact information for a colleague that can be called in an emergency situation will be provided to you if Brooke Randolph will be unavailable for an extended period of time. In the case of an emergency, if you are unable to reach Brooke Randolph, please contact your primary medical provider or the nearest emergency room. If you are feeling suicidal, support is available 24 hours/7 days per week at 1-800-SUICIDE (784-2433) or 1-800-273-TALK (8255).

What are the limits of confidentiality?

Will my information ever be shared with anyone other than Brooke Randolph? Your confidentiality is always a priority in this process. Your identity, personal information, and the topics you discuss will be protected with the highest standards available. Without your written permission, information about you shall not be shared. Even after written permission is granted, it may be revoked at any time. If there is concern of danger or harm to yourself and/or others, Brooke Randolph is legally required to report this information to the authorities to ensure the safety of all. In the case that you are threatening harm to yourself, Brooke Randolph may be obliged to contact loved ones to provide protection and/or seek more intensive treatment. Brooke Randolph may also be required to take protective actions if it is believed that you are threatening harm to another. These actions may include notifying the potential victim, contacting police, or more intensive treatment. Brooke Randolph is obliged by law to report any suspicion or allegation of abuse or neglect of a child, elderly or disabled person.

Some clients elect to schedule coaching appointments in public locations; Brooke Randolph is unable to ensure that information discussed during these meetings will not be overheard by others. In such cases, you are responsible for the information that you choose to discuss.
